Do I need a physician’s orders to be eligible for home health care?
Yes, you will need to have seen your PCP, surgeon, or specialist 90 days prior or within 30 days after the start of care before receiving home health care. A Home Health Face-to-Face Encounter document will have to be signed by your health care provider, requesting care for you to be in a home health setting. Home Health Care requires a homebound physical status as a necessity to qualify for services.
Who are the members of your staff?
All Seasons Home Health and Palliative Care is composed of licensed nurses, home health aides, physical therapists, occupational therapists, speech therapists, and medical social workers. We also have case managers who can guide clients in acquiring home health care services.
How soon can I receive care after I send a request?
It normally takes about 24-48 hours for us to arrange for a nurse, home health aide, therapist or medical social worker to be assigned to your case. Throughout the process, our case managers and staff at All Seasons Home Health and Palliative Care will always be there to guide you.
