What is Personal Assistance Services or PAS Services?
PAS or Primary Homemaker Services provides essential services so that our patients might succeed in maintaining semi-independent lifestyles without having to abandon the comfort of their own home.
It sounds great but how is this achieved?
The needs of our patients are carefully and individually evaluated to determine how many hours and what types of service will be required each week.
Yet how might one determine if our services would be of benefit?
You might find that your loved one has recently been discharged from a nursing home, hospital or ER with little or no caregiver support during the day and this impedes the ability to perform such basic tasks around the home that the average person takes for granted, even in simple things like remembering when to take medications. Additionally, some will lack the mental condition to cook and clean, while others will perhaps possess the desire to do so but lack the physical strength or energy. All Seasons Home Health and Palliative Care are pleased to offer care services which include but are not limited to grocery shopping; laundry; picking up prescriptions from the local pharmacy; healthy meal preparation; escorting to doctors’ and other crucial appointments, and also assisting with personal hygiene. What is Alzheimer’s Disease?
Alzheimer’s is a type of dementia that causes problems with memory, thinking and behavior, Symptoms usually develop slowly and get worse over time, becoming severe enough to interfere with daily tasks.
Alzheimer’s is the most common form of dementia, a general term for memory loss and other intellectual abilities serious enough to interfere with daily life. Alzheimer’s disease accounts for 60-80 percent of dementia cases. Alzheimer’s is a progressive disease, where dementia symptoms gradually worsen over a number of years.
Although current Alzheimer’s treatment cannot stop Alzheimer’s from progressing, they can temporarily slow the worsening of dementia symptoms and improve quality of life for those with Alzheimer’s and their caregivers.

Cardiac Rehab Program
This is a conditioning program which assists the cardiac patient and family with the transition from the inpatient setting to home. It enables the homebound patient to continue the rehabilitation started in the hospital. In conjunction with their physician, the PT and Occupational Therapists recommending various activities to increase the patient’s strength, save their energy, relieve their anxiety and enable them to do more for themselves. when ready, the patient will be referred to out-patient Cardiac Rehabilitation with their doctor’s approval.
The objectives of our All Seasons Home Health and Palliative Care Cardiac Rehab Program is to:
- Evaluate patient safety, home assessment, need for assistive devices
- Guide the patient to increase activity
- Reinforce patient awareness of Target Heart Rate (THR), instruction in body mechanics and energy conservation techniques
- Instruct in relaxation techniques and Mental Imagery
- Facilitate full participation in Activities of Daily Living (ADL’S)
- Teach low intensity aerobic and anaerobic exercise in both supine and sitting positions as per PT recommendations
- Reduce patient and family anxiety levels
- Provide a bride for a continuum of care after the inpatient stay and while awaiting outpatient Cardiac Rehab
Care Transitions
Ensuring patient care needs for a successful outcome is our priority. Our Care Transition team coordinates among all health professionals involved in order to ensure a smooth transition that is as comfortable and safe for the patient as possible.
Services include:
- Obtaining all necessary paperwork form the hospital and coordinating/scheduling primary care physician to follow up visit
- Coordinating any necessary medical equipment and home care services such as nursing and therapy
- Reviewing medication information and communicating with nurses to help ensure accuracy
- Following up with the patient within 24-48 hours upon returning home to ensure all needs have been met by our staff
- Evaluating the patient home for any safety risks or concerns
- Providing assistance with effectively communicating health care needs
Congestive Heart Failure
In some cases, CHF can be managed with the help of medication, proper exercise and a healthy diet. Through our caregivers and home health aides, you or your loved one can lead a more active lifestyle and overcome daily obstacles. What is Congestive Heart Failure(CHF)?
Heart failure describes the inability or failure of the heart to adequately meet the needs of organs and tissues for oxygen and nutrients. This decrease in cardiac output, the amount of blood that the heart pumps, is not adequate to circulate the blood returning to the heart from the body and lungs, causing fluid (mainly water) to leak from capillary blood vessels. This leads to the symptoms that may include shortness of breath, weakness, and swelling.
Causes of CHF
Existing conditions like Coronary Artery Disease, Diabetes, and Hypertension(high blood pressure). Cigarette smoking, obesity, lack of physical activity, a poor diet high in fat and cholesterol and low in fruits, vegetables, and grains, previous heart attacks, and congenital heart defects.

What is COPD?
Chronic Obstructive Pulmonary Disease(COPD) is an umbrella term used to describe progressive lung diseases including emphysema, chronic bronchitis, refractory (non-reversible) asthma, and some forms of bronchiectasis. This disease is characterized by increasing breathlessness.

What is Diabetes?
As explained by the Diabetes Research Institute, to answer that, you first need to understand the role of insulin in the body. IV Home Infusion Therapy Services
This program will enable the patient to be discharged from the hospital, receive IV antibiotic therapy at home and prevent unnecessary admissions to skilled nursing facilities.
Home IV Antibiotics & Hydration Therapy
All Seasons Home Health and Palliative Care are able to accept Medicare patients to administer IV therapy in the home setting following Medicare guidelines. The patient will be managed by our SN IV nursing therapy team.
All Seasons Home Health and Palliative Care will work with the infusion therapy company in the coordination of services to ensure proper, scheduled delivery of the IV medication(s) and supplies.
All Seasons Home Health and Palliative Care will follow scheduled IV medication administration as ordered by MD.
